This is going to be a bit long, but bear with me please.

So after finishing Act 3, I decided to check the internet to ensure that I hadn't missed anything, and apparently you're given an opportunity to join the voidwoken and become sworn to the God-king. In my first try, I killed both Alexander, the Shadow Prince, the Mother Tree, AND the Sallow Man, after he turned on me. When I went to the voidwoken, I (Sebille) could only convince Fane NOT to join - I could not tell him to join, nor was I given an option to join myself. That sucks, because I'd actually really like to join as my character, but can't for the life of me figure out why I'm unable.

I haven't found any actual definitive discussions anywhere online as to what is required to become sworn - I've heard many varying things; it's not even clear to me whether it's the Sallow Man or the Voidwoken in the academy that actually give you the offer. Regardless, neither asked me, so, knowing I must be missing something, I went back for a second go at the Nameless Isle, trying various things I read online.

This time around, I killed the Shadowprince, the Mother Tree, and Alexander (did not speak to his spirit.) I made sure to use the black mirror, was given directions to the cave, and brought the severed head there. What followed was varied:

If I completed the quest as my party leader, I was told by the narrator that "You know that he [Sallow] has something that you seek. Something important." Immediately after, without exiting the dialogue, he: asks me if I am sworn, tells me to swear to the God-king, and says he'll give me the key to the academy/mountain in exchange. However, I'm not actually given the option to join him - I can only either kill him there, or ask him how to become sworn, resulting in him trying to kill me (saying I have to be dead to be sworn?) Thus, if I complete the quest as my leader, Sebille, killng Sallow is unavoidable.

Since I read that he must be alive when I visit the voidwoken, I couldn't let that happen. So I tried to complete the quest as someone other than Sebille. In this case, in the final line of dialogue with Sallow, the narrator says "you know that he [Sallow] has something that you seek, but will only share them with the person who leads your party", and the conversation/quest ends there. Now, if, as an undead character, I speak to him again, he says "Serve the God-King. Reap his reward." If I speak to him with Sebille, or any living character, I am forced through the same dialogue tree mentioned above to fight him.

No matter what, in the journal, under the now 'Closed' quest "The Sallow Man", it says in the final entry: "It seems he has something we need, something important . . . but he won't fulfill his end of the deal just yet . . ."

This suggests that, if you can indeed become sworn through the Sallow Man (and I've not seen any actual evidence other than hearsay that you can), he will only give you this proposal if your PARTY LEADER is undead. Can anyone confirm or deny this? lol. Either way, can someone with an undead party leader tell me what it is that you can get from him, if not entry into the covenant?

Point being, the Sallow Man was a dead end. Next I thought to see whether I could get the voidwoken to give me the proposal instead. Since I could not kill the Sallow Man to get the conductor, I took the alternative passage to the Academy (some people were saying that this was necessary to receive the offer, anyway.)

Either way, even with the Sallow man still alive, the voidwoken would not ask me to join. It simply asks Fane, and goes nowhere with any other character (even if they are undead - I tried.)

I am unsure if this is related, but there also seems to be a bug with Sallow's reward - after accepting one of the tag changes, nothing happens. The dialogue simply ends, and there is no change to my tags. I saw in Let's Plays that he said some stuff after.
