In DoS 2 the best items you can get are likely going to be the highest level items sold by the vendor. There are a few exceptions, like that shield which gives permanent deflection (and can be found in the room leading up to saving arhu) but otherwise, once you reach level 21-22, the gear sold by vendors will outscale most unique gear due to the way stats scale. Its kinda sad that unique gear is lackluster, but it is what it is. With that being said, the unique items I found in A4 were the following:
An Maflin (Unique Shield), Found in the room leading up to Lord Arhu in the Vaults of Kemm.
Kemm's Deflector (Unique Shield), Found on the body of Lord Kemm.
Dumora Lam (Unique Wand), Found in the Doctor's demiplane, only obtainable with lohse in your party.
Falone Girt (Unique Amulet), Found in the chest on the boat in the harbour.
Ruvola (Unique Chest), Found in the chest in Lord Kemm's Vault.
Kallisteis (Unique Boots), Found in a vendor, on the cathedral side of the bridge.
Falone (Unique 2H Axe), Found in the cathedral.
Lucian Divine's Staff (Unique Staff), Lord Arhu's Chambers.
Domoh Dumora (Unique Dagger), Balcony of Lord Arhu's Chambers.
Deiseis Riveil (Unique Bow), Vendor on Arx Square.
Vord Emver (Unique Crossbow), on the body of 1 of the adds in the fight against the Sourcerer trapped in the cage in the sewers.
Voor d'lamas (Unique Two Handed Mace), Warfare Merchant on Arx Square.
Anathema (Unique 2 handed Sword), Tarquin, quest reward for helping him restore Anathema, only received upon discovering he resurrected Braccus Rex.
Voor d'Aravel (Unique 1 handed Sword), obtained in the Consulate, in the chest at the top.
Ancient Spear of Bloodletting (Unique Spear), found behind the consulate on the coastline, can only be reached via teleport type skills, on the corpse of the voidwoken.
Confiscated Ancient Empire Water Staff (Unique Staff), The Magister's Vaults.
Ednet Voor (Unique Spear), The Magister's Vaults.
Luronta Temva (Unique Pants), Equipment Merchant.
Dora Rav (Chest Armour), found in the death room right before the final fight.
Then there are 3 items which are labelled as common, but are kinda unique:
Calamity (Malady's Staff), obtainable in the dream realm, found in the consulate.
Brahmos' Blade (2 handed Sword), obtained from the dream realm as well.
Ghostly Morningstar (2 handed Mace), obtained from defeating the Doctor as a quest reward for saving the spirit tree, the quest in Act 2. This weapon is kind of interesting since it does piercing damage and as far as I know is the only weapon in the game that scales as piercing. I would consider this the, "infinity sword" of dos 2, since if you can kill the doctor, who is the most armoured enemy in the game, you don't need this weapon for anything.
I probably missed some stuff and my list is by no means exhaustive, just the items I found during my playthroughs of the game.
