To be honest I don't think I've gotten any of these bugs in my co-op playthrough, though we are only halfway through Act 2. Many of these sound like they could be caused by desync between the host and the client(s).
I think the only bad bug we've had is one where it was nobody's turn and the fight could never progress. Sounds like your #8, actually.
The single-player experience is definitely more robust than the co-op though.
