I'm unsure how (but it was likely from unchaining/rechaining party members or/and from having NPCs join/leave), but the skillbars for certain characters became locked somehow. Repeatedly. Give us an option to toggle their default state.

Thankee!

Their default state? As in, the first time you look at them? I think they stay whichever way you put them but I don't know. I mean, is it a big deal? The icon is right there. Then again maybe it is because I keep spitting curses at the game whenever I try to move something because I compulsively lock it.

I've unlocked my skillbars more times than I can count because, for some reason, the game kept (re)locking them.

Seconded.

Tiny yet surprisingly annoying issue. The skill bar always reverts to locked upon reload or starting new session. Considering the fact that every new consumable/usable item is *automatically* added to your skill bar...

Yup, I'd love being able to set a default option for this behaviour as well. It's a minor inconvenience and there're certainly more grave things to fix currently, but it would be much appreciated for the future. At the very least, allow us to prevent every picked up item from sneaking in between our skills.

If skillbars were unlocked by default, people would complain about skillbars unlocked by default and them losing a skill from the bar. Tricky, isn't it?

I don't care about the click from time to time, but I also don't care about achievements (what's the sense of them?) and use a mod since long to avoid items entering my skillbars.

Not really, no. Just prevent it from defaulting back to locked once changed. Problem solved.

Yeah, I will probably do the same once I finished my first playthrough. I generally prefer to abstain from applying any mods until then. I don't care much about generic achievements either, but sometimes they can be nice as they might point out different ways of how to handle a situation that you might not have realized otherwise. Or hint at some secret places / possibilites / choices. Otherwise I guess they just offer a sense of progression to people akin to the leveling up process itself.
