Same bug here. I found out on reddit, that it's supposed to happen when you put the stone tablet into your stash on the lady vengeance (the one she asks you to bring to her for the oathbreaker quest).
Nevertheless, this bug is really annoying, especially for people that play on honour mode and DON'T have a save to go back too. :/
