You can insert images anytime by using the "enter an image" function in the post form.

But this is one of the more infamous bugs that has gathered a lot of reports. I'd say this is pretty normal. Good for me it didn't hit me in my playthrough.

"How" is an interesting question. The short answer for the question is, because combat is not ended properly. I would like to know the technical answer to that too. Because as I see it, it should be super easy to prevent something like this: the moment one of your characters interacts with the portal, immediately kill off / destroy all of the eruptions, then put the "end combat" command there, right before transporting your team to the next area. Pretty simple, right?
