So this happened when I tried to access my journal:

Try going to Map and then back to Journal as a workaround. The problem goes away after Act 1 for some reason.

I'll try that the next time it happens. I had to reload the game for it to go away. This happened while I was in Driftwood and I was worried I would have to finish the rest of the game with a malfunctioning journal.
