Activating, not actually using the item, this started happening with the most recent patch of 12-6-17.
Others have reported it skipping there turn, but it is because if you have 2 AP and for example, click an arrow, just activating used up your 2 AP.
You can also activate and cancel repeatedly to use up all your AP without doing a single action.
Regular skills seem unaffected by this though.
