You can do it once per character, plus once for each different shapeshifted character which makes a total 5 items each. With a bit of save-scum you can get 5 Epic ring or belts, with +2 to whatever you want, for each character. The item type (belt or ring) seems to be a fixed order but the bonuses are random and the level is tied to you character level.
