I didn't have any characters who could read the witch's notes (Alice and the two cows/humans) so I cheated and looked up the recipe. However, it didn't work (I'd say what they are but it might be a spoiler). Has that changed since the classic OS2 (ie, is the formula I found out of date?)? Does one actually have to read the book to get it to work?

You shouldn't need to be able to read the book in order to create the potion (Witch's Eye + Boletus mushroom + Augmentor).

Interesting; I got the dreaded red "x" when I attempted that. It doesn't take a higher level of augmenter, does it?

The funny thing is that, me being me, when the question came up at who was better suited to be returned to human form, my mind said, "which one of you is prettier?". Then I changed the submissive one and the human form was male. smile I guess they changed their names after becoming cows. smile

No, just a regular augmentor.

If you email supportdos2@larian.com with your save (or a Dropbox or Google drive link, etc) I can take a look at it. If you are using any non-Larian mods, also include a list of those enabled and/or the modsettings.lsx file from your profile folder.
Each save is a folder in the '..\Documents\Larian Studios\Divinity Original Sin 2 Definitive Edition\PlayerProfiles\<ProfileName>\Savegames\Story' folder.
In Windows Explorer you can zip a file or folder by right clicking on it/them and selecting 'Send To | Compressed (zipped) Folder' (and similarly in OSX/Finder).

I've been waiting to try it again but have not found any more empty potion bottles. Some vendor should get them in again at some point and Hans is looking for one so I should soon know.

Well, it works without the bottle. It really should require a container, though, like all other potions.
