When I have done the meister's ritual and after going into the pseudo hall of echo I had to go afk for a bit and then when I come back everything looks fine until I use spirit vision. When I go to talk to Amadia she gives me the dialog you get if you come back before getting more source slots, but without the smoke to come back.
Here is a link for a resave after teleporting Fane back in the developer build. I'll create a bug report to get a programmer to take a look at this (it seems the smoke exit disappears after some time).
