I suppose we'll find out in just under 3 weeks (fingers crossed), but it occurs to me that we've only seen Astorion making feeding choices when he was the character played by Swen. We've also been told how it will work in multiplayer. If you're playing a different character and Astorion is in your group, how do you suppose they'll decide whether (and on whom) he feeds? Will they even tell you (it's not like they tell you which of your party members takes the devil's offer)? If not, I predict a bunch of bug reports or complaints about not healing properly after a long rest...

As stated in the latest update, when Astarion wants to feed at your camp, all the players will see the dialogue that comes with it. And so they can vote whether they want themselves or someone else in the party to be fed on. No surprise here. They'll know exactly who the victim was.

The OP was asking about how feeding works if Astarian is a party member who is not the main character in single players.

I would hope that companions have their own story arcs rather then "playing through" a designed-for-a-player content. So there should be something designed for player(s) to learn about Astarian feeding habits (maybe wake up to Astarion feeding on one of the companions?), with intention to make in engaging and interactive from player point of view.

Unless they make a point of you waking up the first and/or every time (and having you influence his target), there's still the question of who he targets (random, does he target certain characters more, does he target wounded or healthy more) and how much he drinks (when you are controlling him there's a chance to botch a roll and accidentally kill after all). That and the question of what you do when you find out (and whether you're the only person who finds out at first, or whether it defaults to everyone finds out). If you find out first and keep the secret, how much grief does that cause when everyone finds out (I doubt it's possible to keep it under wraps indefinitely). For that matter, can he feed on enemies? Does the "feeding while camped" mechanic get bypassed if you have him scout ahead, sneak up on some goblin and drain the bugger?

Oops sorry, misread the question. I would assume you don't notice it if it's not your character who was the victim. But there must be some indication something went wrong like health shortage or mood. It would be cool if the character who was attacked was in a weird mood the next morning.
