After I had a pretty full inventory and had been moving things between characters' bags, I noticed that the icons started overlapping each other on my main. When I'd move the top-most item another item would move to take its place, and the one beneath to begin with would stay. I would have to repeat this about 4/5 times to get to the underlying item.
