When you level up and click the plus button on the portrait of the character, the accept level up button is basically right on top of it, so if you are not fully paying attention, you can accept a level up accidentally. I am playing on 3440x1440p if that changes anything. In this resolution, there is also a lot of blank space in the middle of the screen.
Should probably put the accept button on the right side of the screen.
