For all things you described there is such thing as "ready action" in 5e.
Some feats allow to use reaction in bit more nonstandard way, like if enemy attack you ally, while you in melee range with that enemy - you can use reaction to hit the enemy.
But using reaction to attack enemy who just enters you threat range is overly OP. Even more so for ranged attacks and spells.
