I was walking through the side path with the exploding gas area right below where the beholder appears and was about to climb the vines and go up. As I was climbing my character slid off the vines and through the terrain and died instantly at the bottom. it then sent my dead body to camp and wouldn't allow my other characters to go to camp so I could revive my character as it said one of my characters was unable to go to camp.
