People get way too hung up in these approval/disapproval reactions. They act as if they had all the same weight, which is beyond stupid.
Chances are most of them are minimal variation equivalent to the character raising an eyebrow, and only a sparse number of cases gets a genuinely strong emotional response.
Also, I noticed that sometimes the reaction is not even about the decision you take, specifically, but about how you choose to word your answer.
I.e. "Fine, I'll help you since you showed me proper respect" - NPC that appreciates you expressing authority approves.
OR
"I'll help you because what happened to you is unfair" - Same NPC: "What a wanker".
