The only case where it would happen is Gale with his scroll of true resurrection. The revivify spells/scrolls has to be used under 1 minute of death in-setting and do not fix body damage. The tadpole is also connected to the other party members and will know they are about to resurrect the dead party member.
That leaves Skellie boy, but maybe he wants you to have a tadpole for all we know, he's not really forthcoming with information.
