It doesn't have to change gameplay at all. It could be a potential ending.
I was thinking that could be a possibility. Perhaps there's a bad ending where he makes you his first vampire spawn ofter he becomes a full vampire (after promising to do the same for you.) Or, if you had maxed out approval with him he makes you a full vampire for murdery adventures together. Epilogue stuff.
