Well, to be honest, it is an example where if you fail the roll, you must reload. You cannot "roll with it", that is accept the failure that alters the story a bit : you get clobbered by the Mind Flayer.

Larian said they want failure to be fun and not require reloading.

I got screwed once. Now I just kill it on sight before any dialogue.

Suggestion : it could get up with 1 to 10 HP maximum, giving the level 1 "party" (often main plus Shadowheart and pets) a chance to defeat the thing... possibly. Or, better, some RP situation where the Mind Flayer recognizes you as a potential Absolute and let's you go. It probably already controls you (failed Wisdom Save) if it gets up anyhow.
