Today' Gale was the king of Underdark bugs.
After patch 3 came out I did not pick Gale for the party so this his first time adventuring. That means he died for the first time in the Underdark. And his "death hologram" cut scene did not trigger, so I revived him with a scroll. Maybe it helps - he was under Sussur Blooms effect when he died first time.
Gale died for the second time some time later, "death hologram" appeared this time. I summoned memphit but despite that I did everything right memphit does not want to bring Gale back to life.
