I don't know about you guys, but I get the dream after the very first time I use the tadpole to compel or dominate (using it on/with/against your companions doesn't seem to count, and in most cases I never did anyway). One (Authority) use, warning about tadpole taking something you'll never get back, then dream at first rest after that. Then, almost immediately, one more (Authority) use, and right away, second dream on the next rest.
For me, before patch 3 the first use was always followed by the "you feel sated" cue and it never triggered the dream. I always got it after the second use, after receiving the cue about "something you'll never get back", as you mentioned. Subsequent uses would trigger the other dreams.
Now I have to use it at least 4 times to trigger the first dream and subsequent uses don't necessarily trigger the other dreams. I don't usually use it with companions with the only exception of Lae'zel after she speaks to Zorru so I don't think it's linked to that. Unless this single companion-related use broke the triggering system...
I found it very weird and I really wanted to know if it's intentional or if it's a bug.
