Something similar just happened to me...
I was fighting the hag and Gale fell to his death. I keep fighting and a few turns later I get a journal update kindly warning me that "Astarion is dead. No time for a funeral." What!? Then it hit me... Gale's corpse. Of course. Damn mage and his necrotic energy. Shadowheart, Astarion and Scratch (!) need protection against this walking bomb. Can we move his tent away from the others please? Or not... since this really cracked me up.
It would be funny, if companions could actually be resurrected after Gale kills them...
Well, I gotta go kill that hag again I guess... thank you Gale.
