Hi guys,
I'm coming here for help before all this get me mad. I try to understand how personnal conversations are triggered with each companion. For all I can tell, either it's totally bugued, or these algorithms were written by the most complicated disturbed mind I've ever met (my guess : both.). I played this EA maybe a dozen times and I have seen Shadowheart trying to open the mysterious box only once, Gale tell me about his story only once, Astarion talk about what he will do when free from the tadpole only once, Wyll tell me about Mizora only on 3 or 4 of my runs (let's say one run on two)... I think you can catch the picture!
So, I decided to make an exhaustive amount of try and carefully report every conversation I am able to trigger and the state of my relationship and story progession for each case. If you have any knowledge that can help me, or just want to do some tests of your own and report them here, you are more than welcome!

Event requiring companion to be in party : That part is pretty bugued. You must have Wyll with you for the fight BUUUUUT! He must be hidden when engaging the fight. If not you will not have the scene where Wyll question the goblin leader about Spike if you keep him alive. BUUUUUUUUUUUUUUUUUUUUUUUUUUUT !!! If you keep Wyll hidden when engaging the fight you will not have the scene when he ask you to keep him alive. So, if you want to have the full scene you have to :
1) save your party.
2) engage the fight normally with Wyll at your side that will trigger the encounter scene when he ask you to keep the goblin leader alive
3) reload your save
4) Hide Wyll before getting closer
5) Because Wyll is in your team (even hidden) the fight will be triggered with no conversation at all
6) engage the fight with Wyll hidden
7) use wyll during the fight (you don't need to keep him hidden all along)
8) resolve the fight without killing the goblin leader (knocking him down or let him surrender when his hp growth low, I don't remember well)
9) Allow Wyll to ask his questions. (If you kill the goblin leader I think you can question his corpse with the "talk-to-dead" spell but never tried)
10) Thank Larian for all these impressive work

This is still a work in progress thing. I don't have trigger everything, my memory is not perfect, all tests were not Patch 4 tests.... Following, and I agree some things appear out of order and/or are VERY glitchy.

I romanced Astarion on my first playthrough, no problems. I tried to romance Gale on my second playthrough, and couldn't for the life of me trigger the Weave scene because his approval jumped too high too quickly. I made sure to Long Rest a lot while he was at Med/High approval and it STILL didn't work. frown Eventually gave up because it got too annoying loading up saved games over and over and experimenting just to try to get his scene.
I don't understand how Gale is the most-romanced companion according to Larian's data, if his very important Weave scene is so difficult to trigger!

I certainly hope this gets fixed and the companion scenes queue up properly after next patch...

I have also noticed that the triggers are... Inconsistent, at best. I do hope to see Larian address this in one way or another - hopefully already in next patch!

Some notes: my party tends to be Lae’zel, Wyll, and Astarion. I rarely swap them out.
I’ve kept this post only to camp scenes, but another day I can list the encounters that cause a “Reaction” from a companion in the field.

Companion: Lae'zel (!), Shadowheart, Gale, Astarion
Subject: Initial Camp Night 1; Cutscene of finding camp location in beginning. You will get shorter or more aloof dialogue with Lae’zel or Astarion if you talk to them last or 2nd to last. Gale is always introspecting no matter what order you talk to him in.
Approval level: Neutral
Story progression: encountered some companions; prior to reaching the Grove

If he is not, a scene with Lae'zel (inspection) will play immediately if she is in the party. Lae's scene is completely glitched at the moment as of Patch 4, but was present in Patch 3. Afterward, the other companions have something to say about not transforming yet.

Additional conditions : from what I can tell, this scene or a similar one will not play without either Gale or Lae’zel recruited

I tend not to have any problems with the first 2 camp scenes, but I always do them before reaching the Grove. The rest of the camp scenes do not trigger at the same times across all of my playthroughs. Some culprits:
  • Zoning – sometimes triggering dialogue seems to only require traveling to a new area where the name pops up on the screen
  • Camp Queue – Currently it feels like scenes that are set to occur after an event will push scenes down the queue so to speak, and require additional camp nights. If two scenes are set to trigger after the same event, one will be overridden.
  • Clearing the Queue – Because of the inconsistency of the triggers or just because you can run through so much of the game while resting very few times, you may need to camp repeatedly to “clear the queue” to get a scene to occur or ensure that it is not overridden.

Have the same sad story with dialogue triggers. I only manage to get the "stars" scene on the third night, before Raphael, recruiting Wyll, talking to Nettie and Ethel. Have no idea what really influences the trigger other than meeting with Zorry. Because when i did all the listed above before meeting with Zorry, and then met with him, the dialogue didn't trigger at all. To get the Weave scene I had to get A LOT OF long rests before entering the Druid Grove, other way it never triggered also.

4 more runs later, I have some improvements to share!
Many thanks to @AvatarOfSHODAN, your Zoning and Clearing the Queue tips help me a lot. I successfully trigger new scenes and was able to trigger them again at each new run. The only one I was'nt able to trigger again is the Shadowheart's "be catched trying to open the box" scene.

Event requiring companion to be in party : That part is pretty bugued. You must have Wyll with you for the fight BUUUUUT! He must be hidden when engaging the fight. If not you will not have the scene where Wyll question the goblin leader about Spike if you keep him alive. BUUUUUUUUUUUUUUUUUUUUUUUUUUUT !!! If you keep Wyll hidden when engaging the fight you will not have the scene when he ask you to keep him alive. So, if you want to have the full scene you have to :
6) engage the fight with Wyll
7) use wyll during the fight
8) resolve the fight without killing the goblin leader (knocking him down or let him surrender when his hp growth low, I don't remember well)
9) Allow Wyll to ask his questions. (If you kill the goblin leader I think you can question his corpse with the "talk-to-dead" spell)

Great list.

About Shadowheart playing with the box: I've managed to trigger it in every one of my playthroughs (so like 4 full playthroughs), but it always comes very late. My typical route is to clear the Underdark before killing off Ethel. I've never gotten the scene before completing some or all of the Underdark. Perhaps it's been moved from patch 3 so that it comes later (and thus foreshadows a certain spoilery scene)?

Some people have had difficulty triggering certain scenes that they want to have, so having this kind of list gives insight into what events were involved in triggering them and which events possibly overrided the desired event. I've had to use guides to trigger certain scenes, which is frustrating. :\

The companion conversation triggers seem pretty odd at times. It's good to share what the triggers are - especially if some of us are replaying the game a million times anyway. Might as well share.
