You mean the entrance to the Underdark via the defiled temple? Find the room with the moon puzzle on the floor. It's accessible via the quarters of the Goblin High priest. After solving it, the entrance opens and you'll find yourself in the Selunite Outpost in the Underdark. This is the moon puzzle room, which is something like the inner sanctum of the defiled temple.

No I mean how do I get to that part? I need a way into the Defiled Temple before I worry about getting into the Underdark .

Just use the main entrance via the Goblin Camp or climb to the left of the main entrance, there should be a cracked wall. You can destroy it and create another entrance to the Defiled Temple.

Specifically where? Do I go through the Sanctum or somewhere else? I can't progress any further without this information. I'm in the Sanctum right now, stood where the Priestess was. Where do I go now?

I don't get it. You stand, where the Goblin priestress was, right? That's already in the Defiled Temple. Now you're looking for the quarters of the Goblin priestress?

The guides all state that in the SW of the Sanctum - to the immediate left of the area of the Priestess - there is a room blocked by an Ornate Door. Inside that room is another Ornate Door that apparently leads to the Defiled Temple. I can bypass the first door by jumping across a little further down but once in the room I can't open the Ornate Door. If that's the correct entrance how do I open that door? If it's not can you please give me detailed directions?

The key to the locked door should be on the priestess' body.

Yeah that's the problem, it's not. I don't suppose there are any alternative ways in are there?

Just in case you haven't discovered this ... you can press Alt key to highlight interactable items around you, just to be sure the key is really not on the table. If it's not on the table, then it has to be on Priestess Gut's corpse. I've never found myself locked out of that room.

It's not on her corpse, I have every Key and I've checked every body several times. It must be a bug and not spawned. It's not like you can sell keys. To clarify, her room is the room behind the first Ornate door? The room you can jump across the gap through the broken wall? The room that has the second ornate door in? Or am I searching in the wrong place?

While it's possible that the game is bugged and the key is not spawned, but in all my playthroughs, I've never not had the key on the priestess. You can see the room in this video

It's not my video, but it starts right in the room we're talking about, and when Gut dies, you can see that the key is on her corpse. Priestess' Key is the name.

Yup, that's the room and she doesn't have a key. Great.

Well, you can still teleport out of the goblin camp. And you can get to the Underdark through other means.

What other means? That's what I've been asking all day. What are the alternative routes, please?

There is a hole in the spider matriarch's lair that looks rather deep ...

The easiest other way I know of reaching the Underdark is

Explore the place. Have you killed the matriarch?

I appreciate the help you're giving. I have no idea who or what the matriarch is. Starting in the main hall of the Shattered Sanctum, the place where the Priestess does the branding, the spider tunnel is behind her throne. I entered that, talked to the spiders and told them to leave. The only transitional exit in here is called 'Cave Mouth'. Taking that I'm in a different area called the Defiled temple but it's a simple leftward curved tunnel leading to a raised floor with no exit beyond the one I entered through. There's nothing there to explore.

Ah, we're not talking about that spider cave. In the Blighted Village there's a cellar (reachable from falling down a hole that's covered in webs or picking the cellar doors; the building is NW of the Eastern entrance to the village). In the cellar there's a wall that can be broken near the anvil/forge area. Beyond that is a cave, and eventually you'll reach the Spider Matriarch in an area with a giant hole.

The matriarch is in a cave that is accessible at two points in the Blighted Village. One entrance is down the well, and the other is through a wall in the blacksmith's workshop. You'll need to smash down the wall.

Get to the matriarch and you'll see the hole. Be prepared for a deep fall and you'll land in the Underdark. You could be sneaky and jump down the hole without fighting her too, I suppose.

Are you level 4, BTW? To kill the spider matriarch, you'll want to be level 4 to have an easier time. And bring light sources.

There are at least four ways to get into the Underdark. They did make the Gut route harder by making the ornate door non-lockpickable, but you can either a) or b) use her key. If you can't find the key, reload and kill her again.

The Matriarch is found when going down the well in the blighted village.

As far as the key goes it may be tied to the conversation arc where you have Priestess Gut help you remove the parasite only to be imprisoned and chained up. I have never killed her any other way so I can not verify that.

Different spider cave. There is another spider cave when you climb down the well in the blighted village. There is a deeeeeep hole you can jump down but need featherfall or you die.
