Some ladders (such as the ones around the bridge to the githyanki patrol) won't let you climb them. I've also noticed this with several ladders now, and it's always the really long ones like in the goblin camp and zhent hideout; in combat, out of combat, it doesn't matter. A character will climb to the top of the ladder and teleport down to the height of the bottom of the ladder. It makes some paths inacessable for characters that don't have a good enough jump.
I've had this bug too, only with some roots. It looks like they had no "exit" points and so characters would climb up only to go down again. And it was not possible to go down those roots from the upper level.
Personaly i believe the problem here is the same as with "cannot reach" objects ... For some reason game presume that most objects are aproximately 1m higher than they actualy are ... therefore from engine perspective, there is no ladder.
I still dont understand why cant we change Race for our hirelings. Lets us play Githyanki as racist as they trully are!

This is hilarious.
It's a bit annoying you can't order your flying familiars & animal companions to pick up those objects. A bird getting the "can't reach" message doesn't make sense.
Yeah also, pathfinding sucks too after patch 5. Some times, for no reason , a companion (no, there is no water in the way of Astarion) just decides to stop and not follow you anymore!
