We can imagine a cosmetic kit as item with special action to trigger the character creation UI with make-up as only parameter to be changed ?
Items can be looted on some specific or available on some merchants inventory.
Idea is to avoid to find a gnoll with mascara into its inventory (fun but not realistic).
