The exact same has been happening to me with cutscenes not working as they should do and companions no longer having the reaction dialogue to the Blighted Village - Interrupting The Ogre Couple & 2nd night at camp conversations with your party, only one party member will have their cutscene/dialogue. It only worked for me with Lae'zel or Gale. Astarion had nothing to say but did have his dialogue in previous patches along with Shadowheart.
I also had multiples of Astarion after first night at camp.
