Anyone experiencing this should ideally submit it as a bug report to their bug reporting form (
https://larian.com/support/baldur-s-gate-3#modal), which is separate from the general feedback form. If/when you do, include your save file with the report (they prefer you to submit the whole folder content - so both the save file, and its accompanying thumbnail image), as that's helpful for them to examine to track down the cause of the issue.
