I do really hope that this will be sorted at the full release and that it will be far harder to miss companion scenes. That said, in my patch 8 playthrough I did think I got more of the Gale and Astarion conversations despite not actually trying for them, though not sure how much that was coincidence or having got in the habit of doing things in a particular order (e.g. long resting at certain points, or talking to Zorru before any other healer leads to get Astarion's stargazing scene) rather than because Larian have fixed anything. I didn't spot anything about this in the patch notes, but there were a lot of them!
Surprisingly, I got one of Lae'zel's camp dialogues when I accidentally clicked her in the wilderness near Scratch... I don't know if that was a bug or if they are implementing companion dialogue/scenes outside of camp.
There is one of Lae'zel's dialogues - one about the Tears - that she will give outside the camp, and then she'll say again next time I long rest so I assume that's a bug. I think I noticed this in a previous patch too. Not sure if this was the same one you came across?
