Just had an interesting fight where I was dual wielding, but unless I selected Off Hand Attack first, my regular attack action will burn my Action and Bonus Action. However, only the Main Hand Attack will actually go off. In the battle chart there's no listing of a "Tav missed with Off Hand Attack," basically only giving one attack per round. Is this a known issue?

When you equip two weapons, dual wielding turns on automatically which means you strike with both weapons when selecting a default attack. There's a tiny diamond-shaped button under your weapons which you can hardly see has two swords on it that you can click to change to default to only striking with one weapon, so you can choose your off-hand attack or other bonus action separately, or I think the default toggle key is "R" if you're on PC.

To be clear, it does do two different attacks and things like true strike only help the first one

If you can take a screenshot of your combat log, and confirm that your character only made a single attack roll, while the game took both your action and bonus action for two-weapon fighting, then that's definitely an issue. If you can take the time, please do consider submitting a formal bug report to Larian's bug form (Here: https://larian.com/support/baldur-s-gate-3#modal), and include with your report your save file (including the thumbnail image file that will be in the same folder; they request both files), and a screenshot or screenshots that show the log and the issue; the more replicatable it is, the better.

It was reported in the past here, yes ...
But it would most certainly not hurt to follow Niara advice and report it.

I have a fighter that dual wields, but I turn it off so I have the option of who I want to hit or use BA in another way. The hand xbows also are duel wielded so same.

I also have this issue. Thought it might be due to mods, but I uninstalled all the mods I had and reinstalled the game and it still occurs. For some reason, this appears to affect the MC, but Astarion seems to be unaffected by the issue.

Here's a screenshot I took of a freshly created barbarian using dual wielding to attack Us, only the main attack appears in the log, but both the action and bonus action have been used. Also, the attack animation did attack with mainhand weapon then the offhand weapon.

It's most definitely a bug.

I notice it a lot of times when shooting [Main Attack] + [Off-Hand Attack] with a single mouse click, but the [Off-Hand Attack] does not display in the combat log. No miss, no hit. Just unregistered completely.

It is a similar issue with the Gnolls trying to get to Rugan. If anyone here has noticed, when they use [Seething Fury] their next attack will not register.

So I'm sure these bugs will be fixed eventually, because these are definitely unregistered hits by the system.

WORKAROUND: Press "R" to toggle single-hit attacks and use Off-Hand attack manually.

Happened to me as well, but not consistently. Will try "R" next time.
