I'll do a "Goldilocks playthrough", as I call it. Not simply completionist, but try and complete quests juuuuust the right way, which is usually a mix between fitting a story narrative in my head, and (especially these days) trying to dodge or workaround the sort of bugs that mess up the Goldilocks playthrough like making sure the owlbear doesn't bug itself back to running around the goblin camp after the 3-4 special dialogues that makes it stay in camp. Once in a while, I'll come up with a new "mission" for myself, like trying out ways to make sure that obnoxious half-elf druid in Kagha's cave gets killed by the Shadow Druids, rather than his more reasonable, but low-HP halfling buddy (which I've yet to succeed with, btw). Or do the most spectacularly satisfying thunderwaves, or blows with that wonderful mallet you can get from one of the goblin bosses.

She is the only stable in my group and yes, I know, you don't like her. But you can just go to where Gale spawns first and collect him and/or free Lae'zel and then go back to Astarion and kill him in dialogue.

oof, that's a rough choice, recruit SH and be able to kill Ass in dialogue, or not recruit SH but cannot kill Ass. No win situation there ^^

Definitively a win situation for me, since Shadowheart is my favorite companion and Astarion my most disliked one.
