It's a common and persistent issue that skipping dialogue during any conversation/cutscene will frequently break and displace animations completely; this has been an issue since day 1 and seems to be an inherent 'feature' of the way Larian's game engine works. You may have been feeling like you're seeing it more on subsequent playthroughs simply because you're fast-forwarding through dialogue more frequently and in more places each play. Keep reporting it wherever you have the energy and time to do so; all we can do is hope they do decide to address this at some point.
