This might have been reported already but when fighting in the underdark at the hook horrors the rumbling earth gets stuck in a loop and knocks the characters down but then an endless loop of the rumbling earth "preparing its next move" without actually moving or showing the monster. It also doesn't lett the characters up from the knockdown.
