With destructible objects and large/small creatures it's very possible to not be able to pick up some items because they can't physically be clicked due to being obscured by other objects. Examples just from my latest playthrough are books I can't get after dropping the ceiling on the ogres in the Blighted Village, and Buthir dying on top of Grukkoh so his items can't be looted (stop snikkering!)...

I only recently discovered that you can click on the Alt key tooltip text to pick up an item or loot a corpse in addition to the item itself. This made life so much easier with things like books hidden under skeletons and piled up corpses. I am not sure if that's new, or if it somehow took me until patch 9 to realise! Does that help with your ... ogre problem? There might still be things without tooltips that this doesn't address.
