PC (GOG) - Hotfix #30, system spec is in my signature - I am not using GOG Galaxy to launch the game, nor am I using the Larian launcher, I just launch the game using BG3.exe. - No mods in use.

At the forge in the Blighted village.

I sparked up the bellows to heat up the forge.

Then interact with the forge :
1. Added Sussur tree bark, clicked Combine - The forge fire turns blue, Tavs voice then says "Now to add a Dagger, Greatsword or Sickle"
2. Added a Dagger, and then clicked Combine ..

The special dagger is not produced, and all I can do is press escape to make the combination dialogue vanish (its own close button does not work)

Is this a known issue?
