This item works sometimes, and others not. I can't seem to find any reason why it just stops working. One thing I have seem to noticed is that when it IS working you will NOT get the 'attack of opportunity arrow' from the NPC that will attack. If they just have a circle then it seems to be ok.

I am going to see if either removing the item and placing back on works (not a SOLUTION.. but might shed light on problem) and also if re-loading helps..

Will edit this if/when I have more info.

Save/Load(a couple of tries) :NO
Unequip/Equip: NO
Sneak away and camp for PARTIAL REST: YES YES
Sneak away and SHORT REST: NO NO

So whatever is causing the pendant to stop working WILL only end on a partial(and I guess full) rest at camp... Um.. only thing i did not test is if just GOING to camp fixes it. But hopefully all this testing will help solve the problem O_o

After over an hour waiting for the game to reload multiple times (loading takes 4eva on my laptop) I am burned out on this bug ):

From this, I wonder if the pendant might only work once per long rest, as opposed to every time your HP drops? I don’t think it says that on the description, but just a guess from your outline of the problem.

Though I also had some issues with the Helmet of Grit last playthrough, which also triggers once HP drops below 50% but sometimes did and sometimes didn’t. Though I never managed to work out what was going wrong there, either, so all I can do is feel your pain!

I have been using Grit for a while during this run and have not had any issues. Also have a thief using it in a different run. (Thief with THREE bonus action is cray-cray, especially when combined with the GameBreaker... oops I mean... LineBreaker Boots. (:

I'm working on a vid showing a squishy bard killing EVERYTHING with Linebreaker,Grit, and the L4 sorc feat to get Expeditious Retreat. When the dang pendant works... it is a thing of true beauty O_o

A bit more info about my Helmet of Grit issue in case it helps with the Moondrop Pendant one. I'd saved a game showing my character wearing the Helmet of Grit, <50% HP and 1 bonus action in order to submit a bug report to Larian. But I have just reloaded this save to check it before submitting my report, and when reloaded my character has 2 bonus actions! Fortunately, I had also taken a screenshot at the same time as making the save clearly showing a single bonus action or I'd start to think I was going bonkers.

But I see you've already tried to save/reload a game with the Moondrop Pendant not working, and it's not helped. So it does look as though the issues are probably unrelated, or at least present different symptoms.
