I'm noticing that after the dream, you can talk to Astarion and win his persuasion check to get +approval. After that, every option I tried resulted in a huge negative approval hit to Astarion except speaking to Lae'zel and then Astarion, then leaving camp. I just spent 45 minutes reloading and trying every possibility and they all end up in a very large approval hit that would not appear in dialogue, but at least a minute after you walk away.

yes its bugged by the script auto selecting your main character being copy \ pasted and not allowing for the party moving... what should happen is [random] party members say something and you can defend Astarion but now its auto select option 1 = bad for you

Speaking of Astation, he is a favourite to romance. And i would love it if we werent tied into being mean to get his approval. It would be cool if as you progress through the acts you have the option to show Astarion good aint too bad. And by the end of the game he is true neutral. An option of careful dialogue choices to change his allingment would be really intresting. And allow players that want to be relativley good aligned to have that option. Astarion is awesome and he has a sad story and can be quite stroppy. When u talk to him nicley he is sometimes shocked your being lovley to him. So I can really see you bringing him out of his anger and misery. Just a thought 🤔

he likes kink as well so you can agree to a bit of light S&M at the goblin camp for his approval... of course you are sort of still being mean but its to yourself

Astarion will approve if you say "Baaa!" to the Redcaps pretending to be sheep (in the swamp).
This even works twice.

When he talks about Cazador (after meeting Gandrel), you get his approval when you choose the option "You'll watch my back and I'll watch yours" (I don't remember the exact phrase, but it was something along those lines).

And in the first night at camp, tell Shadowheart that you shouldn't rush through unfamiliar territory.

There are other opportunities I can't remember right now to get his approval with "non-evil" options.
I choose the "good" options for my Tav, but I still get Astarion's approval high enough.

Update for the current patch, if anyone is interested: I choose the "good" options in my playthrough and play a Tav who will help others, but there was no problem to get Astarion's approval to High. I was very (pleasantly) surprised that he did not even turn my Tav down when she took the initiative at the celebration.
I had already braced myself for his very harsh rejection 😂
