Sex is prevalent in our lives, extreme violence is not (for most of us). The former subject matter is more real - we can actually do that stuff if desired, the latter just fantasy (outside of war, which is described as a nightmare). We all have personal sexual preferences and standards, there are things we like and things that cross our line. It's not hard to empathize that some people found parts of the sex scene unacceptable to them.

The game is done, the PFH shown, can't change anything. The only thing open to the offended is to post their dissatisfaction. The sheer number of people offended should indeed invite some reevaluation. I note that I can't find the latest PFH on Larian's Steam page, but I can see their December one. It'll be telling how Larian decides to publicize this PFH.
