Had the exact same bug. I eventually figured out that one of duergar I had pre-killed before blowing the tunnel was not dead, Only unconscious with 1 hp from a pommel strike. Reloaded to before I blew the tunnel, finished the duergar and then blew the tunnel and everything went normally after.
