If you notice that you're igniting surface effects without meaning to when you walk around them, you're not going crazy. I was doing it a lot last night ,and couldn't figure out why.
Check if you have a torch equipped and not active. Example: torch in melee slot, but you have ranged slot active. As crazy as it sounds, that was igniting oil, etc when I would walk close to it. Super frustrating, but at least now I know what is going on.
