Nope, not the only one but I'd not spotted your earlier post. I actually managed to get a screenshot of this yesterday, but I'm still trying to work out if there's any pattern.

Here's my screenshot, and in it you can see that Shadowheart is the currently active character in combat, and has shared initiative with Wyll, but the next two characters in the order are greyed out as they for some reason had their turns before Shadowheart and Wyll.

In case it helps work out when this is happening, this is the fight in the shadow cursed lands ...


against the drider and pals. In my playthrough, I'd used the rogue option to let them pass (hoping that I'd have a cunning option to attack them from behind), but the Harper leader had asked me wtf I was doing and leapt out at them, starting combat.

"You may call it 'nonsense' if you like, but I've heard nonsense, compared with which that would be as sensible as a dictionary!"
