I'm almost sure it happens every time an enemy spawns minions. I think it has a great chance to occur when the player's group is split; for example, to position the companions for an ambush.
It's not the only bug, by far, but it ruins the combat quite often because we can't predict who's next in the queue.
I can't imagine why this bug is still there after so much time in EA - I've seen reports of this during EA already (but since there was no bug report system and we were actually told not to report them, it may have been ignored). What's done is done, but I hope they have this on their radar and can fix it quickly.
I've made a couple of bug reports about it, complete with one save file (the limit makes it impossible to attach more) and screenshots. I encourage anyone to save the game when this happens during a fight and to report the problem too. More data will make it easier to pinpoint the bug.
I've seen several patterns (no effect justifying that someone should be skipped):
- companions being active in a different order than what is shown in the queue
- companions that are skipped; only two of them are active and the system switches back & forth between them and the enemies
- without doing an 'end of turn', a pop-up appears to tell it's not that companion's turn (like below), and that for each character during the combat
