There is a scene you can trigger in camp where Orin kidnaps a companion, and she will pose as that companion holding Yenna hostage to trick you into telling her to slit Yenna's throat. If you fail the persuasion check to inquire more to reveal the companion is in fact Orin, Orin will slit Yenna's throat and gloat. It may be that the game bugged out and the game state advanced and assumed Orin killed Yenna as disguised as a companion but you missed the camp scene.
If Orin directly kidnaps Yenna, though, she will kill and cook Grubs and make you eat him before she reveals herself. The good outcome option, at least what worked for me, is to let her kidnap Laezel and catch her impersonating Laezel and persuade her to release Yenna. You then talk to Yenna before restig, she's traumatized, but after the rest she will become a soup seller at camp. Grub always replies "Please don't look at me." if you have animal speaking.
You can then do what you need, and Lazael won't die to Orin as long as you don't trigger progression by encountering the murder tribunal or going to her temple. If you do any of these, she will kill the kidnapped victim if you take some long rests in between. So if you kill the murder tribunal, try to have some Angelic Potions saved to give your characters that need spell slots a long rest substitute without actually resting and then go bust up Orin.
You always need to pass a dice check 25 in the dialogue to get Orin to not kill your companion during the dialogue if you didn't fulfill her request and killed Gortash before visiting her domain.
The best way to get Orin to not kidnap a companion you don't want kidnapped is to have them in your party. The pool of companions that can be kidnapped are Lazael, Halsin, Gale, and then if those three are in your party, she will default to Yenna.
