We're talking about regular NPCs though, not party members.

Naturally for party members we just open their inventory and equip it, but NPCs which wear underwear and are not part of the camp yet; such as Minthara/Halsin/Jaheira who can die before joining the camp will not drop their own underwear when killed. They have to be knocked out first for the underwear to appear as loot, otherwise it's lost forever.

OK, I misunderstood. I'm only in act 2 for the moment, but I saw the trailers, and I want red Orin's underwear for my trophy cabinet.

I do wish there was better variety of camp clothes in Acts 1 and 2. Story-wise I guess it makes sense that there isn't, but could enemies drop some or something?

Unfortunately no. They wear the same default clothes as a Tav.

He doesn't have a brother. But he does have a sister. She is named Arka.

My point was it doesn't matter. Arka can sell the clothing, or it could be Jamie Lannister. There's not a lot of narrative legwork needed to justify why a clothing vendor is in the grove.
