Patch 3 seems to have broken DUrge’s ending choices in certain circumstances.
Before patch 3, DUrge always had the option to choose whether to seize the brain in the name of Bhaal or betray Bhaal and destroy the brain, but as of Patch 3, DUrge only gets the option to seize the brain. I’m not sure if this is specific to Orpheus being a mindflayer, but it prevents DUrge from getting the madness / suicide endings unless you have Gale to sacrifice.
