This is something I commented on a lot during early access. The game leans into even or neutral game play. I did my first playthrough as a Bard that wanted to do good but was not adverse to what many considered Evil if it helped him do good later. I ended up with a "good" outcome but many times had to resort to an evil feeling method.
I am doing a play through right now as a Paladin of the Triad. (Good Guy) I created a custom Cleric of Selune as a companion and am using Gale and Wyll as my companions. Yes I know Wyll has a Devil granting him power but he wants out of the deal and did this thing for a greater good. I want to help the young man be set free. I had to kill Astarion, I mean a Vampire tried to bite me in the middle to the night. Karlach is dead because Wyll thought she was a monster and Paladins of Tyr told us she was. (Found out later we where wrong so have had to pay penance for the transgression but we where tricked) Then of course cannot take Shadowheart as she worships Shar.
This "Good" play through has been MUCH harder than the previous playthrough. I will not consume the worms so never get those powers and do not use them when I have the option. This means much tougher fights, much more challenging. The sad part, is there is no reward for this effort. Feels like the game gives you more rewards for being evil and with an easier game play.
