So I think this is a bug but in all my previous playthroughs iirc Vlaakith cutscene triggered when I kill the Inquisitor and his gish or when I give the Artifact, but in this current playthrough the Vlaakith scene doesnt trigger either way and the Journal and Map Marker is stuck with Return to the Inquisitors Chambers, I've reloaded couple times and did every possible dialogue combination and the Vlaakith scene wont trigger locking me out of furthering this part of the story, does anyone have any idea on what else to try?

P.S : I have Mods, but they are only cosmetic, I have no mods that change gameplay and If these mods were the culprit then I wouldnt be able to progress at all on my previous playthroughs.

I am running no mods and having the same issue. My inquis is alive so I will try killing to see if that does anything. I have a slew of other issues after this patch. Might need a rollback on this patch until it stops breaking the game.

EDIT: YES, after the failed trigger when I produced the artifact for the inquisitor, killing everyone triggered the Vlaakith convo.
EDIT 2: AAAAAAND now I'm softlocked and it won't let me use the planecaster. Can't go in the artifact, can't interact with the right statue to open the secret passage to the mace. RIP.

RESOLVED !!!

I had been researching this bug for days. I had tried every suggestion I had read to fix this. I had no luck with any of it. I had triggered the Vlaakith scene in previous play-throughs, and could do so by loading those saves, so the only variable had to be something different specifically about this play-through.

The problem was Volo's Ersatz Eye.

In other play-throughs I did not let Volo replace my eye. In this play-through I had allowed him to do so. I loaded a save from before the eye surgery, rushed back through to the Inquisitor scene, and Vlaakith triggered properly now that I did not have the magical eye.

I hope this helps others who have been frustrated by this bug.
