There be some more spoilers below (no more than yours).

There appears to be a rather large disconnect between the Alfira execution and the scene where you wake up your love interest. Yes, you get to talk to your butler, but you never get to inform your companions about any of it, such as his tasking you with the murder of Isobel. In a resist Dark Urge playthrough, it almost makes sense that your love interest would react this way because it's been so long since Alfira with no incident that the excuse of it being the tadpole almost makes sense. Hells, they weren't even more than a reluctant travelling companion at the time that you brutally murdered Alfira and they got together with you anyway.

There should be more dialogue between these scenes where you can warn your companions that while you are trying to resist, the butler - and the urges themselves - are urging you on to do these things. But then again, this way the reply after waking them up would have to change, too.

For what it's worth, I can warn you to contain expectations regarding Dark Urge in act 3. It will get a fair amount of extra content, but companion replies are not always as in character as one would hope for due to bugs. This may vary depending on the companion.
