Context:
Act 2 ( before going after nightsong)

I pursued a romance with Karlach
I was also very friendly with Shadowheart
After Karlach got her second upgrade
I had my first romantic night with Karlach, but as she indicated she has an off switch and an on on on switch. My character didn't feel comfortable with that so I decided to spend the night with her lying next to each other. Also because at that point I was actually more interested in Shadowheart as a romantic partner. Now the next morning Shadowheart commented that I looked very happy but she was just regretting she was not the source of that happiness. I then indicated to her that I was actually more interested in her. However, the option I got then for declaring my love for Shadowheart was very cold (it's done, as in I'm broken up with Karlach), I would've wanted something more nuanced, but my fear was that with games like this (with choices and consequences) many times you get just one chance. That's the reason why I chose the cold option because I wasn't sure whether I would get another chance with Shadowheart.
What I suspect was intended is that you could break up with Karlach first and then you could come back to Shadowheart to say "it's done", but that option shouldn't be available until you actually broke up with Karlach. It should've been Shadowheart saying something like "well, perhaps you should go talk to Karlach about this first" after you tell her you want to be with her more.
The bug:
Now, I saw the exclamation mark above Karlach's head but I ran into Shadowheart first (that's the reason I talked to her first) and then I wanted to talk to Karlach. However, from that moment on (and I'm in Act 3 now) Karlach still has that exclamation mark above her head and when I want to talk to her she simple says stuff like "I have a new best friend now" or "I guess I have to admire you from a distance" but I can't actually talk to her anymore. So that's where I think it's bugged.
